  • Annual Wellness Exam (includes Diagnostic Testing)

  • Personalization and Preventative Care

$85/mo (auto-draft)

Junior Membership
39yrs and younger (over 14rs of age)

  • 24hrs access for Semi-Urgent Needs

  • Annual Wellness Exam (includes Diagnostic Testing)

  • Personalization and Preventative Care

Each office visit will be a $30 copay (not including injection administration).

Please be advised that our office does NOT take insurance. You may still use your insurance for other needs outside of the office: Labs, Imaging, Referrals, etc.
